Wray Community District Hospital, a critical access hospital in Wray, Colorado, provides medical services to patients of Northeast Colorado, Western Kansas, and Western Nebraska. The hospital participates in the Centers for Medicare & Medicaid Services Primary Care First (PCF), which allows Wray to support patients with extensive care management programs. When it first offered care management programs, over 35% of the eligible population had uncontrolled diabetes with A1c levels over 9%.2 To help address the population’s diabetes challenges, Wray implemented Oracle Health Care Coordination Intelligence to help identify diabetic populations and assigned a full-time care manager to focus on diabetes education and treatment. Through regular outreach and education, Wray steadily reduced the uncontrolled diabetes population from over 35% in 2018 to 14.55% in 2024.3
